How to Submit Your Data in an Assessment

Visit the Proof V10 landing page and login to your account. Learn how to login to your account.

  1. After you receive an assessment invitation, your home page should display the notification prompting you to complete an assessment; Click ‘Complete next task’ to begin working. Alternatively, you can click the Assessments icon outlined in red on the left side menu to access the assessment.

  2. In the assessment, respond to each prompt following the indicated instructions and click ‘Next’ at the bottom of the page when you’re ready to move on. You will have a chance to review your answers before submitting.

    Note on Assessment Questions: Questions with a red asterisk are required - you will not be able to click ‘Next’ without entering a response in the correlated field. Many of the questions follow a similar pattern; the first question in a set will gauge the relevance of the question to your organization; if relevant, the assessment will next ask if you have data for that question and proceed to data entry. The subheader under each question will contain relevant information - be sure to read this closely when submitting data.

    All questions should have an optional free response box where you can explain any details related to your response, elaborate on your organization’s planned activities for data collection, or write how your organization plans to improve on its results in the future.

    The assessment should automatically save in real time. You can exit the assessment and return with your progress saved anytime before the due date.

    Note: Only one person from your organization can work on the assessment at a time.

  3. You can navigate the assessment by clicking ‘Next’, as seen in the image above, or by clicking ‘Previous’ outlined in red on the image below. On the final page of the assessment, you should see the option to preview your responses. Click ‘Preview’ to continue.

  4. You should now be able to review your responses. Click ‘Edit’, outlined in red on the image below, on any responses you would like to change. When you are satisfied, click ‘Complete’ to submit the assessment.

    Note: After you click ‘Complete’, you should be able to reopen and edit the assessment, as long as it's before the submission due date. If you need to edit responses after the submission due date, you will need to reach out to [email protected].

  5. You’re all set! Click ‘Back to Assessments’ to return to the assessment homepage, or the Proof icon in the top left corner to return to the platform homepage.

    Note: You should receive automated prompts from the system to submit data for future assessments in line with your organization’s reporting cadence (monthly, quarterly, bi-annually, annually).
